The 9 subdial works as your first explanation - there is a hole in the dial, and the rotating disc is fixed to the movement.

In terms of the hands, I have seen Seafarers and Mareographes with a third variation of hand, the early style of polished hand but with a black central stripe. As to the case markings, I have seen both with and without the Autavia marking on the back, I commented this on another thread. A good example is the "Haslinger" Skipper with this style of case, near NOS, which had no Autavia marking on the back. That was a very cool watch, but anyway, it's gone to someone else!
