Just for clarity, I emailed Anthony today.
He reported that the first insert had got lost in the post (I think that's what was meant, as the email was a little garbled - I think predictive text may have been at fault)
And another insert is being sent to me, as of today.

Anthony, I mean no angst towards you, I am only reporting the facts.
Like I've said before, I have bought from you before, with no problems whatsoever.
I just don't know what has happened recently.
