does anyone know of a source for
: replacement leather bands for a Tag Heuer
: Carrera 1964 re-edition?

In addition to Nick's I know one other source of these perforated bands from Lonestarwatches. They offer only the black 18 mm which is the size the 64 Re Issue uses. I have purchased from both of these fellows and have had great service.
