Looks like a standard Carrera 45:
From the 1964-1966 Catalog...

There is also a white dial Carrera 45 pictured at the top of the Carrera directories in the Chronograph section of OTD (also pictured below) although that seems to be the only picture Jeff's posted (thus far).

You should be able to get the model number (3647S) off of the catalog page, and you might wish to ask Jeff if he has more pictures of the plain white dal Carrera 45...
