OTD contributor Alex (APE) shows a similar dash chrono on his great website Breitling-Navitimer.com which you should check out. Your dash piece appears to be the model 615. Go to the opening page of Alex's above mentioned website and scroll down a bit to see the Breitling 615 timer/chrono. Then, look at the top of the page and left click the "Dash" icon to see a few more pics.
