Warranty card, owner's manual, list of warranty service locations, and if your watch is a chronometer you get a small certificate stating so.
Serial number will be on the watch and the outer box (though sometimes the outer boxes get accidentally swapped, so go by the number on the watch).

There is no specific certificate proving the dealer was authorized. Your receipt is that proof. With the name of the dealer and date of purchase on the receipt, Omega can verify that that dealer was an authorized at that time.
