I have a watch in need of repair that I'm going to send in also. So I contacted Omega and here's there response:
We thank you for your message regarding your Omega Speedmaster watch, which needs to be repaired.

The parcel should be sent by registered and insured mail, together with a "certificate of registration" form no CF 4455, which can be obtained
at any Customs Office, to avoid difficulties upon the re-importation to your country.
Please make sure that your parcel is clearly marked "watch for repair/customs clearing by receiver in Bienne, Switzerland".
Upon receipt of your watch, our technicians will examine it very closely and we shall submit you our report with all relevant details (such as costs, payment, time length of the repair etc.).
