The most common cause is mishandling while at a dealer. Especially with a chronograph, where customers and salespeople may play with the functions roughly or drop boxes while moving things around.
I doubt that this is very common or due to them leaving the factory in non-operational condition. But I have no info beyond that to quote or guess at statistics. Certinaly it is in the best interest of OMEGA to minimize the number of watches that have to come back for warranty repairs immediately after being sold.
