If you are sending it to the US Service Center in NJ, DO NOT send any original boxes, warranty cards, etc. with the watch. I made that mistake and ended up having to wait for them to be returned to me (they had to search their warehouse to find them). Ship your watch in another box that you don't need back, since your watch will be returned in a small padded Omega zippered case. Include a letter in the box that states the exact nature of the problem and contains all of your contact information. Also include a copy of your dealer stamped and dated warranty card or sales receipt from the authorized dealer you purchased it from. You DO NOT need to send in the originals like I did (again I had to wait while Omega USA looked for them - I got them back a week after I got my watch back).
Upon receipt of your watch, Omega will mail you an invoice that will show what they plan to do to correct the problem and if there is any cost to you (i.e. if it wasn't a mechanical defect but something that you did like dropping the watch on the floor). In my particular case, they fixed the problem AND did a full servicing on the watch (the $300 full maintenance package, which included a full polishing job) at no extra charge.
