I presume a CPB form is the same as a CF 4455
John Rochowicz wrote some instructions for another forum member a few days ago.
Maybe John or Houstonwatchfan can provide you with some more Info. I'd E-mail them.
Maybe they can help you.
I'm NOT saying that someone WILL do this, but if any of our American posters have a CF 4455 form that is filled out & would care to scan & send me a copy, I will gladly hide the personal Info & host the form so that we can direct people to it in the future.
I live in Germany & I know that when I sent my watch to Switzerland, I spent nearly 1 hr in the Customs office filling out the German form. ( there are fields on it like "goods number" which is an 8 digit number... you simply can't guess these numbers ! )
