I have not yet sent a watch to Switzerland myself, so I found out about CF 4455 from other posts. However, I have shipped a lot of packages overseas and have always used US Customs form PS Form 2976, which I have found in every post office I have ever been in. Someone can correct me if I'm wrong, but I believe this form will suffice. Instructions for filling it out are on the back.
