I live in Europe, so have never see or used this form. But from what I've read you have to take this to your local Customs office to get it "certified" or stamped or whatever.
If you look in my previous post, the 3rd link & on the left is a list of steps. Step 1 states Link 1 has a link in it to find your local Customs office.
Those guys there MUST know what they want to see in the form. Afterall it is THEIR form. I'd print 2 copies off the interent, pre-fill 1 ( & hope its correct ) if it's not, they'll let you know.
