This is a first for me. I not aware of anyone else having this particular problem on this forum or on TimeZone.
By your description, you seem to properly lock the extension edge under the center pin in the clasp. I'm guessing that either some component got bent out of shape or the clasp was marginally defective fron the start. I had a defective clasp myself, although the problem was not the extension but the springs controlled by the pushbuttons. My watch was very new then and my dealer replaced the clasp at no charge.
I saw a number of ways in which the clasp could be damaged. Being less than confident, I purchased another spare clasp from Omega USA in PA. The cost last October including shipping was a little over $80 USD. An unneeded expense, perhaps, but now I'm covered.
