Omega only keeps those things for about 5 years, so you are probably out of luck on the 94. The PO however, all you need to do is take a picture or scan of the warranty card and cosc card and email them to Omega at aftersales@omega.ch and request the Certificate for your watch. You will need to include your name, what you are specifically asking for, and your return mail address also.
